Массив для автозаполнения (autosuggest)

Нужно выбрать из БД данные и сделать такой же массив как в примере. У меня выводится только первая строка массива, как вывести остальные?
Ниже привожу код из скрипта.
Кто может подскажите пожалуйста как сделать...
